17 yo female, previous PID, Sx of nausea, vomiting, RLQ pain

Total size of structure: 8mm.
Dx?
Acute Appendicitis
- US shows noncompressible, fluid-filled, blind-loop in RLq, fecalith (in a different patient) shown on right.
- Graded-compression US is especially useful in ovulating women (35% false-negative laparotomy rate in that population).
- Laminated wall with target appearance of = 6mm in total diameter on cross section is 81% specific for appendicitis.
CT Correlation
- Images above showed enlarged, inflamed appendix w/ obstructing fecalith.
- CT is 87-100% sensitive, 89-98% specific for appendicitis.
- Appendicolith may only be seen in 25%.
